Application Process Guidelines

The proposal should have a cover letter of no more than three pages that includes:

 
  • The primary mission, objectives and goals of the organization
  • A description of the program, including evidence of need, number of people served and how success will be measured
  • A specific dollar amount and how any approved grant funds will be used
 

In addition, the proposal must contain:

 
  • Copy of IRS tax Exemption letter under Section 501c(3) of the Internal Revenue Code
  • An itemized program/project budget showing expenses and income
  • A current annual operating budget that lists income and expenses
  • A list of current funding sources
  • Most current audited financial statement or 990
 

If your agency received a grant from the Teleflex Foundation for each of the past 3 consecutive years, you are ineligible for another grant for one year following the last grant. You may reapply after that off-year.

 

The Teleflex Foundation does not fund the following:

 
  • Endowment funds
  • Hospitals and Medical Centers
  • General operating funds
  • Capital campaigns
  • Memberships
  • Sponsorships
  • Direct grants or scholarships to individuals
  • Labor, athletic or fraternal groups
  • Gifts to organizations that qualify for tax exemption for religious purposes and organizations that are faith-based
  • Goodwill advertising
  • Fundraising events
  • Subscription fees or admission tickets
  • Trips or tours
